Bracket provides two black RJ-11 phone jacks labeled LINE and PHONE for connecting the incoming telephone line and a pass-through handset or device.
Component side includes discrete capacitors, resistors, transformers, and other circuitry along with an Agere chipset; the main controller IC is marked 1648C-TV5 SEGV 0603S 4332024 00 AGERE, and a white silkscreen strip near the center reads IC56A Stratitec.
Reverse side of the PCB carries white barcode stickers labeled IC56A with UPC: 888533100406 and an additional UPC: 661110095756, plus a Made in China marking and a small OK quality stamp near the mounting bracket.
Card is shown resting on an anti-static bag with yellow ATTENTION Contents Static Sensitive Handling Precautions Required labeling; overall color scheme is green PCB, silver bracket, and black connectors as pictured.

Component side is clean with intact capacitors, headers, and RJ-11 jacks, and the gold PCI edge fingers are bright with only slight insertion marks. Card has not been installed or function tested, so modem connectivity and driver compatibility are unknown; includes the Stratitec IC56A PCI modem and anti-static bag only, with no cables, driver discs, manuals, or original retail packaging.
